TupeloLife chronic care program achieves hypertension control rates to meet HEDIS performance goals

DALLAS - May 18, 2022 -- TupeloLife, a digital therapeutics company, will be showcasing their hypertension control program at the 2022 Case Management Society of America (CMSA) National Conference in Orlando, FL on June 1-4.  The company's hypertension control program is proven to achieve HEDIS quality goals for blood pressure control and medication adherence among its participants. This successful program demonstrates the effectiveness of remote patient monitoring (RPM) and case management in improving patient outcomes.

"TupeloLife is committed to helping healthcare providers improve the quality of care for their patients at home. Our hypertension control program is just one example of how we are making a difference in people's lives," said Rob Sanchez, Chief Operating Officer of TupeloLife. "With the help of our case management team and remote patient monitoring technology, we are able to provide tailored care for each individual and drive impactful change resulting in improved biometric health outcomes.  We are seeing remarkable results in patient satisfaction and bending the cost curve."

The hypertension control program is designed to remotely monitor blood pressure levels and provide case management support for patients with uncontrolled hypertension. As a result of meeting HEDIS goals, TupeloLife DT's hypertension control program has been recognized as an industry leader in providing quality care to patients with uncontrolled hypertension.  TupeloLife offers targeted programs for hypertension control, diabetes management, heart failure, and chronic kidney disease management for both RPM and chronic care management (CCM) programs.

The programs are designed to meet the quality metrics outlined in the Healthcare Effectiveness Data and Information Set (HEDIS), a measure used by healthcare payers to assess provider performance. The presentation will take place starting on Thursday June 3rd at 3:30 PM.
